If you find a location or facility that needs attention or repair, or have a safety concern, please notify a Facilities & Grounds Coordinator or Assistant by completing and submitting the form below. Be as detailed and specific as possible in the description of the issue. Our goal is to keep our facilities and grounds in the best and safest condition possible. Please remember you may be the first to spot a maintenance need, so your input is greatly appreciated. Thank you!

If you have an emergency request, do not use the online maintenance form. Call the parish office during regular business hours, after hours contact the Facilities and Grounds Coordinator or Assistant. If you are not able to make direct contact with a staff member, please contact the chair of the Facilities and Grounds Commission. All contact information is listed below. However if we are in transition, but most up to date contact information can be found in our bulletin.
